“All I Want for Christmas” Raffle Promo

MECHANICS:

1. For every P2,000.00 worth of single-receipt purchase of Melawares, Perfect Dining, Bestware and Urban Kitchen products or combination of any of the said brands in participating stores nationwide, customer is entitled with one (1) raffle stub.
2. Promo period is from November 16, 2015 to December 31, 2015. Deadline of submission of entries is until December 31, 2015, closing of store hours.
3. Draw date will be on January 14, 2015 4:00pm at CDC Manufacturing Corporation Main Office, #8 University Avenue Extension, Brgy. Potrero, Malabon City with the supervision of a DTI representative.

5. Present receipt/s to Melawares, Perfect Dining, Bestware and Urban Kitchen Promodiser of the outlet where the purchase was made to get a raffle stub. Fill-out with complete name, address, contact number, email address, birthday, name of store and affix signature.
6. Drop the raffle entries at the designated drop boxes available at participating Melawares, Perfect Dining, Bestware and Urban Kitchen store outlets nationwide where the purchase was made.
7. Participants can only win once. If the name is drawn more than once, the prize with a higher value will be awarded to him/her.

TERMS AND CONDITIONS:
1. Winners will be notified through registered mail and via phone call.
2. Prizes should be claimed at CDC Manufacturing Corporation Main Office, #8 University Ave. Ext., Brgy. Potrero, Malabon City. For provincial winners, prizes will be sent through a courier service.
3. Prizes are transferrable but not convertible to cash.
4. The winner should present the original customer’s copy of the raffle stub and a valid identification card (I.D.) with picture (e.g. Company, Driver’s License, SSS, etc.).
5. Prizes must be claimed within 60 days from the receipt of the notification. Otherwise, it will be forfeited in favor of CDC Manufacturing Corporation with prior DTI approval.
6. The prize tax will be paid by CDC Manufacturing Corporation, where applicable. All expenses, aside from the above prize inclusions shall be accounted to the winner.
7. This promo cannot be availed in combination with other existing promotions.
8. Employees of CDC Manufacturing Corporation including their relatives up to second degree of consanguinity and affinity are disqualified from joining the promotion.
